Report where you filled it
The FDA tells us which manufacturers are shipping. It cannot tell anyone which pharmacy actually had it. That half of the picture only exists if the people who find it say so.
The half no dataset covers
Everything else on this site comes from one federal dataset that describes 132 manufacturers, not pharmacies. It can tell you that seven companies report shipping a 20 mg tablet. It cannot tell you that the store on the other side of town had it on Tuesday, and no public dataset anywhere can.
That gap is the whole reason people end up calling ten pharmacies. A report costs fifteen seconds after a trip you already made.
The report
What happens to it
It goes to a person, not a feed
Nothing you submit appears on the site today. Reports arrive as email and are read by hand.
Only the chain and the ZIP area survive
When fill reports do become visible, they will show the pharmacy chain and the first three digits of the ZIP. Never a street, never a store number.
There is nothing else to leak
Every field on that form is a fixed choice or a number. There is deliberately no free-text box, so there is nothing to accidentally over-share and nothing for us to have to moderate.
When not to report
Not a refusal
We only collect successful fills. Recording that a named pharmacy turned someone away is a different kind of claim, it ages badly, and it is unfair to a pharmacy whose stock changed the next morning.
Not second-hand
If you did not fill it yourself, you cannot know what was actually dispensed — the manufacturer may not have been the one you assume.
Nothing about you
No name, no full ZIP, nothing about your prescription or your prescriber. The form is built so that this is not a rule you have to remember.
